To ensure you receive accurate and effective treatments for your unique skin makeup, please complete the appropriate form provided below and bring it with you to your skin care appointment.
*Recommended for new Elegant Skin clients, and clients who have had a change in skin characteristics or skin care regimens.
Your Skin Health
Elegant Skin requests that each client complete our basic Skin Health form to let us know what type of skin care regimen you follow, what products you are currently using, and if there are any medical concerns we should be aware of when providing skin care treatments and services.

Waxing Release Form
To receive Elegant Skin’s premiere waxing services, we require all clients to complete a waxing consent form prior to their appointment start time. This form not only helps us recomend the best skin care treatments and approaches to skin care, but it also ensures that you are the most comfortable during and after treatment.
